The Center to Advance Palliative Medicine (CAPC) presents "Lessons from a Pediatric Palliative Care Practice Serving as a Medical Home for Children with Complex Chronic Conditions"  webinar.

The number of children living with complex chronic conditions is increasing and pediatric palliative programs are struggling with how to deliver services that help patients and their families live successfully at home. This session will explore how one program has developed an advanced medical home for pediatric patients with palliative care needs through close collaboration across settings.
